When notifying the Coast Guard of an official flight, you must submit Appended Form 2 with details about the operator, employer, and any vessel or commissioning government agency. If you are contracted by a government agency, you must also attach proof of that contract.

Requirement as structured

一前条第一項第一号から第三号まで、第七号及び第八号に掲げる事項並びに次に掲げる事項を記載した別記様式第二号の通報書イ公務操縦者の氏名、生年月日、住所及び電話番号ロ公務操縦者の勤務先の名称、所在地及び電話番号ハ小型無人機等の飛行を委託した国又は地方公共団体の機関の名称、事務所の所在地、担当者の氏名及び電話番号（公務操縦者が国又は地方公共団体の委託を受けて小型無人機等の飛行を行う場合に限る。）ニ船舶の名称、船舶番号等、船種、船籍港及び総トン数並びに当該船舶との連絡手段（公務操縦者が当該船舶に乗船して小型無人機等の飛行を行う場合に限る。） 二公務操縦者が国又は地方公共団体の委託を受けて小型無人機等の飛行を行うことを証明する書面の写し（公務操縦者が国又は地方公共団体の委託を受けて小型無人機等の飛行を行う場合に限る。）
