The application for an operational authorisation shall be based on the risk assessment referred to in Article 11 and shall include in addition the following information: the registration number of the UAS operator; the name of the accountable manager or the name of the UAS operator in the case of a natural person; the operational risk assessment; the list of mitigation measures proposed by the UAS operator, with sufficient information for the CAA to assess the adequacy of the mitigation means to address the risks; an operations manual when required by the risk and complexity of the operation; a confirmation that an appropriate insurance cover will be in place at the start of the UAS operations, if required by an enactment .
