法第百三十二条の十三第三項の指定は、前条に規定する事項を記載した書類（以下「使用条件等指定書」という。）を申請者に交付することによつて行う。 ２　前項に規定する使用条件等指定書の様式は、第二十九号の七様式のとおりとする。
