This includes your name, address, pilot certificate number, the date/time/location of the event, permit numbers, drone registration and certification details, flight plans, and descriptions of any injuries or physical damage.

Requirement as structured

Pursuant to Article 132-91 of the Act, a person who operates an unmanned aircraft must report specific details to the Minister of Land, Infrastructure, Transport and Tourism when an accident or serious incident occurs, including pilot details, license number, incident details, aircraft registration and certification numbers, and injury or damage summaries.

法第百三十二条の九十一の規定により、無人航空機を飛行させる者は、次に掲げる事項を国土交通大臣に報告しなければならない。 一無人航空機を飛行させた者の氏名及び所属する会社その他の団体がある場合にあつてはその名称 二無人航空機を飛行させた者の住所 三無人航空機を飛行させた者の無人航空機操縦者技能証明書番号 四報告に係る事態が発生した日時及び場所 五許可又は承認を受けた年月日及び当該許可又は承認の番号 六無人航空機の登録記号、型式、製造者及び製造番号 七無人航空機の機体認証書番号 八無人航空機の使用者の氏名又は名称 九出発地及び到着予定地 十飛行の目的及び概要 十一報告に係る事態の概要 十二人の負傷の概要（前条第一号に掲げる事態の場合に限る。） 十三無人航空機の損壊概要 十四その他参考となる事項
