第二百三十六条の七十四　法第百三十二条の八十五第二項又は第四項第二号の許可を受けようとする者は、次に掲げる事項を記載した申請書を国土交通大臣に提出しなければならない。 一　氏名、住所及び電話番号、電子メールアドレスその他の連絡先 二　無人航空機の登録記号（第二百三十六条第一項第一号又は第二号の規定による届出をして無人航空機を飛行させる場合にあつては、同条第四項の届出番号。以下同じ。） 三　飛行の目的、日時、経路及び高度 四　飛行禁止空域を飛行させる理由 五　無人航空機の機体認証書番号（法第百三十二条の八十五第四項第二号の許可を受けようとする者にあつては、無人航空機の機体認証書番号又は無人航空機の機能及び性能に関する事項） 六　無人航空機を飛行させる者の無人航空機操縦者技能証明書番号（法第百三十二条の八十五第四項第二号の許可を受けようとする者にあつては、無人航空機を飛行させる者の無人航空機操縦者技能証明書番号又は無人航空機の飛行経歴並びに無人航空機を飛行させるために必要な知識及び能力に関する事項） 七　無人航空機を飛行させる際の安全を確保するために必要な体制に関する事項 八　飛行させる飛行禁止空域に応じたリスクの分析及び評価の結果を踏まえて講ずる措置に関する事項（法第百三十二条の八十五第二項の許可を受けようとする場合に限る。） 九　その他参考となる事項
