無人航空機を飛行させる者が、４－２に掲げる基準に適合しているかどうかを申請前に確認の上、「無人航空機を飛行させる者に関する飛行経歴・知識・能力確認書」（様式３）を作成し、申請書に添付すること。・これまでに無人航空機を飛行させる者が無人航空機操縦者技能証明を有している場合は、技能証明書番号、区分、限定事項（種類、飛行の方法）を記載することで、様式３の添付及び確認事項（飛行経歴、知識、能力）の記載を省略できる。
